Neighborhood Health Association (NHA), a Federally Qualified Health Center (FQHC), is the largest community health center system in Northwest Ohio with partnerships that include ProMedica, Mercy Health, and University of Toledo Medical Center. NHA has grown from a single location in 1969 to 12 medical and dental clinics throughout Lucas County.Today we operate multiple health centers including pediatrics, adult medicine,dental services, health care for the homeless, women's health center, senior centers and a full service pharmacy with lab services on site. Our services are acutely responsive to the needs of everyone throughout the communities. Under immediate supervision, the Medical Assistant performs administrative and clinical functions to assist and support providers and medical staff in the provision of medical care which includes patient triage, and maintaining patient records in an accurate, organized and readily retrievable manner. The Medical Assistant is a member of the PCMH Care Team. All members of the PCMH Care Team will cooperatively work with/interact with the patients and other team members to coordinate patient care and achieve the stated patient and PCMH Team goals and objectives. PCMH goals and objectives include increased access to patient care, high quality patient care, and patient self-management of care. PCMH Duties and Responsibilities includes, but are not limited to: *Attends and participates in the pre-visit Team Huddle. *Attends and actively participates in the monthly team meetings. Clinical Duties: *Greets patients in person or on the telephone; answering or referring phone inquiries *Prepares patients for the visit by directing and/or accompanying them to the examining room; instructing the patient on the PCMH delivery of care process; providing examination gowns and drapes; helping patients position themselves for the examination and/or treatment; and arranging examining room instruments, supplies, and equipment *Verifies patient information by interviewing the patient; reviewing and/or obtaining and recording medical history; taking and recording vital signs, height, weight; confirming purpose of visit (patient's chief complaint) or treatment, and recording all updated information in the patient's Electronic Health Record *Supports patient care delivery by helping health care providers during examinations; *Collects and prepares laboratory specimens or blood draws for diagnostic testing, logs and prepares specimens *Performs basic on-site procedures such as laboratory tests (urine dip, finger stick, pregnancy testing, etc.), dressing changes, and preparation/administration of medications or injections, as directed by the provider *Sterilizes medical instruments and equipment *Authorizes drug refills to pharmacies, as directed by the physician provider *Documents all patient encounters and records all examinations, treatments and test results in the EHR (Electronic Health Record) according to policy, procedure and standard. *Keeps equipment operating by following operating instructions; troubleshooting breakdowns; maintaining supplies; performing precautionary maintenance; requesting maintenance; calling for repairs.
