Upper Chesapeake Health Peer Recovery Coach (Project HEART) in Havre de Grace, Maryland

JOB SUMMARY: This role is an integral member of the team designated to conduct outreach and engagement with pregnant and post-partum women that are engaging in substance use. This role is part of the health team to provide peer support and motivation to engage and encourage patients impacted by substance use with the goal of improving birth outcomes for mother and baby. The Peer Recovery Coach is responsible for working with the patient to develop and implement recovery plans driven by individual patient needs and identified recovery capital. The Peer Recovery Coach will assist in completing referrals to recovery support, treatment, and early intervention/home visiting programs for pregnant and post-partum women. Services will primarily be provided in the community, including meeting patients in their home, outpatient offices, or other community settings.

MINIMUM KNOWLEDGE, SKILLS AND ABILITIES:

Education & Training: High school diploma or equivalent is required. Must obtain Peer Recovery Specialist certification through the Maryland Addiction and Behavioral Health Professionals Board within 18 months of hire and participate in continuing education programs.

Work Orientation & Experience: The candidate must be actively engaged in his/her own recovery with a minimum of three (3) years of demonstrated personal recovery from alcohol and/or drug use. Previous experience with involvement with the Department of Social Services, Early Intervention services, or other family support organizations, and/or professional experience working with OB/maternal child health programs is preferred. A valid driver's license and access to reliable transportation for travel throughout the community to meet with patients is also required.

Skills & Abilities: The incumbent must be able to read, interpret documents and write routine reports. Basic computer skills are required. The Coach must demonstrate excellent interpersonal skills, the ability to relate to patients and health professionals, as well as good problem-solving skills. Must be able to demonstrate patience and tact when dealing with patients, families, and other staff, to integrate Continuous Quality Improvement principles for service and organization work improvements. Ability to work with people of all social, economic, and cultural backgrounds; be flexible, open-minded and adaptable to change, to develop collaborative relationships with physicians, families, patients, interdisciplinary team, and other community agencies. In addition, the candidate must demonstrate a willingness to learn and an interest in acquiring new skills.
