Otter Tail County Transition Coordinator in Fergus Falls, Minnesota

Otter Tail County is accepting applications for a full-time Transition Coordinator position. The purpose of the Transition Coordinator position is to work directly with delinquent juvenile clients at home, work, school, and in the community. Transition Coordinators prepare juveniles and their families for a return home after an out-of-home placement (reactive) as well as work with juveniles and their families to prevent an out-of-home placement (proactive). They are responsible for coordinating multiple services, engaging in comprehensive case planning, and serving as a pro-social mentor for our troubled juvenile clients while operating the Teen Leadership Program. Examples of Duties: Respond as needed for emergency contact. This includes but not limited to Probation Agents, law enforcement, school officials, clients, and client family members. Complete comprehensive case planning with a validated risk/need assessment as a guide, ensuring the juvenile, juvenile s family, and Probation Agent are involved. Support healthy juvenile and family engagement. Promptly respond to messages to ensure a pro-social mentor approach and transport juveniles as necessary. Guide juveniles to develop prosocial activity through sports, arts, employment, etc. Enhance life skills of juveniles by educating on finances, shopping, laundry, self-care, etc. Develop professional relationships with school officials, treatment providers, and other stakeholders. Guide development of prosocial peer networks with our juveniles. Establish and maintain cooperative agreements, facility memberships, program alliances, and community support. Remain current with knowledge and changes in the field of corrections. Work a non-traditional and varied schedule to best support juveniles. Maintain timely chronological case notes and prepare quarterly reports. Perform other duties as assigned. May assist with duties and responsibilities as outlined in the County Emergency Operations Plan and Continuity of Operations Plan. This position is regularly scheduled to work 40 hours per week, including evenings and weekends. The 2020 starting salary for this position is $18.22 per hour, depending on qualifications, with advancement to $27.32 per hour.
